Certificates are available for persons who were born or died in Clay County.

Appropriate documentation, including picture ID must be presented with the completed application.

Genealogical Services:
Birth and Death Records for Clay County start in the year 1882.

Notice to genealogist:
1.      Complete one (1) application for each request
2.      All Birth and Death requests will be mailed.
3.      *Research will be limited to one (1) hour per visit

Clay County Health Department (CCHD), Nursing Office provides a variety of services.

Immunizations:

Adult Immunizations
TD (Adult Tetanus)                                      $30.00 – 19 Years & Over
TDAP (Adult Tetanus, Pertussis, Diptheria)  $40.00 – 19 Years & Over
Hepatitis B                                                  $45.00 per shot
TB Test                                                       $10.00 (Adult & Children)

Please Note:
Immunization fees are subject to change due to cost of serum.
We are no longer able to immunize children with insurance coverage.

Emergency Preparedness:

Clay County Health Department is the primary coordinating agency for the Public Health and Medical Services Emergency Support Function #8 of the Clay County All Hazard Comprehensive Emergency Management Plan.   Developing processes to dispense medications to residents of the community within a short period of time; writing procedures for issuing orders of isolation and quarantine; providing education and promoting awareness for pandemic influenza and other public health emergency topics are some of the issues that are addressed in the planning process.

      Length of time for completed inspections vary and will be done as E.H.S. schedule allows.
